Dear Patient,

We are writing this letter to inform you that we will no longer be able to serve as your pain care physicians at Regional Pain Care Center. After 30 years, we have decided to close our practice. After 30 years, we have decided to close our clinical practice, Regional Pain Care Center, and our surgery center, Pain Care of North Texas, on July 31, 2024. In accordance with Texas Medical Board Rule 165.5, we are providing this notice to allow you to obtain your medical records or arrange for the transfer of your medical records to another provider.

The last day we will see patients at Regional Pain Care Center will be Wednesday July 25, 2024. The last day for procedures at Pain Care of North Texas will take place July 19, 2024. However, we will continue to operate through July 31st to ensure you have ample opportunity to obtain your medical records or transfer your medical records to another provider, as well to answer any questions you might have concerning this change.

Your medical records are personal and confidential. Only with your written permission may a copy of your medical records be released to you or to another physician. Please contact us before July 31, 2024 to arrange for the transfer of your medical records at (214) 509-9691 or you may obtain a copy of the Authority to Release Medical Information at the RPCC website, www.paincare.com and fax a completed copy of the authorization to (214) 509-0947.

We highly recommend patients transfer care to the Pain Treatment Institute (“PTI”) as a trusted site for continuation of your pain management needs as they provide high quality care with the latest pain management techniques to ensure you get to live a more pain-free life. PTI has multiple locations which include Frisco, Plano, Sherman, Mesquite, Rockwall and an upcoming location in McKinney. In the event you have not contacted us about your medical records before July 31, 2024, PTI will become the new custodian of your medical records. You may contact Pain Treatment Institute at (214) 817-4225 or via email at contact@painti.com to obtain or transfer your medical records at any time after July 31, 2024. Requests will be filed as per time limitations dictated by the state law retention requirements.
